  Case summary
  On April 16th, Anzhen Hospital invited Chinese-renowned internationally renowned cardiac surgeons, Prof. Ye Jian, director of the cardiac surgery and transplantation center of St. Paul Hospital, Vancouver, Canada, to perform related minimally invasive transcatheter aortic valve implantation and related academic exchanges. Professors Meng Xu, Zhang Haibo, and Zhou Yujie’s expert team jointly completed a case of difficult transapical TAVI surgery.
  Patient data
  The patient was a 71-year-old woman diagnosed with severe aortic valve insufficiency based on echocardiography. However, the patient was unable to tolerate surgical flap replacement because of poor lung function. Therefore, he chose a lower risk transcatheter aortic valve. Enter surgery.
  Professor Ye Jian is the world's first cardiac surgeon to successfully perform transapical heart valve replacement and is a user of J-Valve? in North America. Transapical aortic valve implantation, abbreviated transapical TAVI, is the delivery of a prosthetic valve through the apical needle to the aortic valve area under the guidance of X-rays. This avoids the need for conventional surgery due to thoracotomy and extracorporeal circulation. The huge surgical risk is to open a small incision on the left side of the chest wall, which is very suitable for elderly patients with high surgical risk, multiple complications, and poor vascular valve disease.
  Surgical procedure
  In close cooperation with Prof. Ye Jian and Ahn Jung’s heart team, the J-Valve? valve with China’s proprietary intellectual property was implanted in just one and a half hours and TAVI surgery was completed. Compared with conventional thoracotomy, it saves a lot of time and greatly reduces the patient's pain and risk.

  Intraoperative zero bleeding, immediate surgical results, severe aortic valve regurgitation immediately improved to mild reflux. The patient successfully removed the endotracheal tube on the second postoperative day.

  Postoperative technical exchange and summary meeting
  After the surgery, Prof. Ye Jian and the experts from Anzhen Hospital had a wonderful and in-depth academic exchange on minimally invasive heart valve surgery. At the TAVI Surgery Symposium, combined with surgical images, TAVI's operation techniques, precautions during surgery, and prevention and treatment of complications were introduced in detail. At the same time, a new technique for transcatheter mitral valve tricuspid valve treatment was introduced.

  Dean Yujie Zhou also attended the meeting. Dean Zhou spoke highly of the J-Valve? valve system and stated that he would vigorously develop the TAVI procedure at Anzhen Hospital and encourage the surgical teams to actively cooperate to increase the number of TAVI procedures. Quantitative changes have led to the transformation of Anzhen Hospital into the nation’s leading TAVI center. Dean Zhou also introduced the future development of transcatheter intervention and encouraged young doctors to learn more and think more.   At the meeting, Director Zhang Haibo delivered a speech entitled "Contemplation of surgeons in the era of valvular heart disease catheter therapy," and presented his opinion on the future development of the surgeon. Director Zhang said that surgeons also need to actively learn to intervene and master catheter treatment skills. In the future, surgeons can use left-handed scalpels and right-handed interventional instruments. Both hands must grasp and both hands must be hard. By learning minimally invasive interventions, surgeons can keep up with the academic front and remain invincible in the future. Taking TAVI as an example, the current indications have been gradually relaxed to intermediate-risk patients. In the future of aortic valve replacement surgery, TAVI will also be the same as conventional open surgery.

  Finally, Dr. Zheng Shuai, the heart surgeon, and Lu Jiakai, director of the Department of Anesthesiology, shared a case of TAVI. The participating experts actively discussed and had high enthusiasm. All the experts expressed their opinions. They had in-depth exchanges with Prof. Ye Jian and Prof. Zhang Haibo. The discussion ended with an extension of one hour from the original scheduled time.
